Evidence of Psychosocial Disability Form
If you’re applying for the National Disability Insurance Scheme (NDIS) and have a mental health condition, you will need to provide supporting evidence to demonstrate how your condition impacts your daily life. The NDIS Evidence of Psychosocial Disability Form is designed to help individuals with mental health conditions access the supports they need for greater independence and well-being.
This form must be completed by a qualified mental health professional:
General Practitioners (GPs) – Can confirm diagnosis, symptoms, and functional impact.
Psychiatrists – Specialize in diagnosing and treating mental health conditions.
Psychologists – Provide assessments of cognitive and emotional functioning.
Accredited Mental Health Social Workers (AMHSW) – Experienced in supporting individuals with psychosocial disabilities.
Occupational Therapists (OTs) – Can assess the functional impact of your disability on daily life.

You may need to ask your regular GP if they can complete the form prior to making your appointment as a longer appointment will be needed. As there is quite a lot of information required by the GP to complete, a support worker or close family member may be able to assist in completing the form by providing some context of how the disability impacts the daily life functioning of the applicant.
